What documents are required to pass the MSEK – Ministry of Health of Ukraine

The Ministry of Health of Ukraine reminds that medical and social expertise is conducted to establish the disability of patients who have reached the age of majority, victims of an accident at work and occupational disease, and persons with disabilities on the referral of a medical and preventive health care institution after carrying out diagnostic, treatment and rehabilitation measures.

When applying to the MSEK, you need to have with you documents that confirm persistent impairment of body functions caused by diseases, consequences of injuries, or congenital defects that limit a person’s normal life activities.

In addition, it is necessary to present:

  • passport or ID card with an extract from the Unified State Demographic Register regarding registration of place of residence;
  • military ID (copy) or temporary ID (for conscripts, conscripts);
  • referral to MSEK (form No. 088/o);
  • medical documentation, originals (extracts from the outpatient card, form 025/o, extracts from the medical history of an inpatient, form 027/o, results of laboratory, instrumental, radiological examinations, advisory opinions of specialized regional specialists, etc.) and the patient’s outpatient card;
  • other additional documents in case of need to confirm one or another cause of disability.

The Ministry of Health also reminds that during the period of martial law on the territory of Ukraine, the procedure for conducting a medical and social examination is simplified. MSEK performs its functions according to the principle extraterritoriality.

In the event that a person applies for the establishment of disability and is unable to come to the MSEK for an examination, the commission may make a decision on the establishment of disability in absentia based on the referral of the medical and advisory commission of the health care institution.

If you or your child has a disability and cannot undergo an examination and issue a referral to the MSEK, the deadline repeated review is considered to be extended for the entire duration of martial law and for 6 months after its termination/cancellation.
